Jax Denver Cleverley was born at rest Sunday, Dec. 23, 2012, at Lawrence Memorial Hospital.
No funeral services are planned.
Survivors include a loving mother and father, Amie and Denver; two brothers, Christian and Joe; and a sister, Myah.
Taken from us far too soon, he will forever be our sweet angel. Jax brought so much joy into our world with every little kick, hiccup and movement.
He will always live in our hearts.
